- Annual maximums of up to $1,500
- You can choose any dentist, no list to worry with
- Payment made on a usual and customary basis

Individual Plans
This coverage is an association sponsored group insurance plan available to individuals and their families. There are a variety of benefit options and coverage may be paid for on a bank draft or credit card basis.
